dc.description.abstract[ES] Resumen: El cáncer de mama es el cáncer más frecuente en la mujer, afecta en su mayor parte a mujeres en plena edad laboral, entre los 45-65 años. Los avances terapéuticos han logrado una muy alta prevalencia (supervivientes) cercana al 86%, pero es la primera causa de muerte en la mujer entre los 35-55 años. Tiene una gran repercusión económica por gastos sanitarios, gastos en prestaciones de incapacidad, pérdida de salarios, pérdida de productividad del sistema económico y pérdidas por absentismo a las empresas. Su tratamiento en general es intenso, prolongado, y origina intenso malestar emocional y una mala calidad de vida que daña el ámbito personal, familiar y social. De forma especial afecta negativamente a la imagen corporal y a la autoestima de la mujer. Tal vez por ello es causa frecuente de largas bajas (ausencia prolongada del trabajo) y de retorno laboral difícil tanto por los efectos del tumor y los secundarios del tratamiento, como por la aparición de trastornos psiquiátricos «menores»: estrés, ansiedad, depresión y trastornos adaptativos. El retorno laboral es más dificultoso cuando aparecen procesos psicológicos y a veces causa más incapacidad que el propio cáncer y sus secuelas. El 36% de las mujeres con cáncer de mama sufre depresión y precisa de apoyo psiquiátrico, y el 55% padece algún tipo de trastorno emocional por ansiedad o trastorno adaptativo, el cual precisa cuando menos de apoyo psicológico y de prescripción de ansiolíticos en alguna de las fases del tratamiento del cáncer. Los factores psicológicos guardan relación directa con la actitud y esta es determinante con referencia al retorno al trabajo. Por otra parte la presencia de ansiedad y/o depresión disminuye la capacidad de la mujer con cáncer para tolerar el tratamiento y sus efectos secundarios o para afrontar los problemas en el entorno personal que un proceso generalmente tan largo y en ocasiones tan agresivo supone. Conclusiones: La aparición de procesos psicológicos en el cáncer de mama se sucede de forma frecuente en la fase final del proceso y dificultan el retorno laboral. La aparición de trastornos psicológicos en la etapa inicial de diagnóstico y tratamiento no es muy frecuente y en contra de lo que pareciera, la mayor gravedad del proceso o la agresividad de los tratamientos no suponen de forma paralela una mayor afectación emocional. Es más, en estos procesos severos la ansiedad o la depresión si aparecen revierten o se aminoran al año del diagnóstico y guardan más relación con características estructurales de personalidad de la paciente y con otros factores del entorno familiar social y laboral que con la gravedad del proceso. Sin embargo es en la etapa final del tratamiento, en un porcentaje cercano al 35% de las pacientes con cáncer de mama, se desarrollan patologías psiquiátricas «menores» que causan severas dificultades en el retorno laboral, cuando no condicionan el no retorno o la incapacidad permanente. Estos trastornos mentales, que acontecen al final del tratamiento, surgen con mayor frecuencia cuando se carece de apoyo en el entorno social o familiar, si la baja (incapacidad) se prolonga más allá del año del diagnóstico, así como en casos de trabajo poco motivador o de carga física intensa o cuando falta apoyo laboral o existe conflicto laboral, y como factores mucho menos influyentes si la paciente tiene antecedentes de patologías o alteraciones psicológicas previas, o edad superior a los 55 años. Por ello conviene identificar y prevenir estos procesos psicológicos para su abordaje completo socio-sanitario y laboral y obtener una mayor reincorporación de las pacientes supervivientes al trabajo, así como mejorar la calidad de vida de las pacientes, y procurar su reintegración a una vida normalizada. Debe de procederse a potenciar el apoyo del entorno, incluyendo el apoyo en el entorno laboral para facilitar la reincorporación al trabajo, incluso con medidas de reincorporación parcial y paulatina al trabajo y/o con adaptación del puesto. También es preciso concienciarnos de que la mujer superviviente al cáncer de mama precisa en muchas ocasiones de la reconstrucción psicológica, y ello también supone el «descanso» terapéutico, el alejamiento del mundo clínico que la atendió en su fase de enfermedad activa y el refuerzo con un periodo de adaptación y enfrentamiento de las realidades vitales. [EN] Abstract: Breast cancer is the most common cancer in women, mostly affects women in the working age, between 45-65 years. The therapeutic advances have achieved a very high prevalence (survivors) of about 86%, but are the leading cause of death in women between 35-55 years. It has a great economic impact for health expenses, expenses in incapacity benefits, loss of wages, loss of productivity of the economic system and losses due to absenteeism to companies. Its treatment is in general intense, prolonged, and causes strong emotional distress and poor quality of life damaging the personal, family and social environment. It especially affects negatively the body image and the female self-esteem. This may be a frequent cause of long casualties (prolonged absence of work) and difficult return to work process due both to the effects of the tumor and secondary effects of the treatment, as well as to the appearance of «minor» psychiatric disorders: stress, anxiety, depression and adaptive disorders. The return to work is more difficult when psychological processes appear and sometimes causes more disability than the cancer itself and its sequels. 36% of women with breast cancer suffer from depression and need psychiatric support, and 55% have some type of anxiety disorder or adaptive disorder that requires at least psychological support and anxiolytic prescription in any of the stages of cancer treatment. The psychological factors are directly related to the attitude and this is determinant regarding the return to work. On the other hand, the presence of anxiety and/or depression diminishes the capacity of the woman with cancer to tolerate the treatment and its side effects or to face the problems in the personal environment that a generally so long and sometimes so aggressive process supposes. Conclusions: The emergence of psychological processes in breast cancer occurs frequently in the final phase of the process and hinders the return of labor. The appearance of psychological disorders in the initial stage of diagnosis and treatment is not very frequent and against what it might seems, the greater severity of the process or the aggressiveness of the treatments does not parallel imply a greater emotional affection, it is more in these severe processes where anxiety or depression if appear revert or lessen at one year of diagnosis and are more related to structural characteristics of the patient’s personality and other factors in the social and work environment than to the severity of the process. However, it is in the final stage of the treatment, in a percentage close to 35% of the patients with breast cancer, that they develop «minor» psychiatric pathologies causing severe difficulties in the labor return, when they do not condition the no-return or the permanent disability. These mental disorders that occur at the end of the treatment appear more frequently when support is lacking in the social or family environment if the disability (incapacity) is prolonged beyond the year of diagnosis, as well as in cases of uninspiring work, intense physical load or lack of labor support or labor conflict, and as much less influential factors if the patient has a history of previous pathologies or psychological alterations, or older than 55 years. For this reason it is necessary to identify and prevent these psychological processes for a complete socio-health and labor approach and to obtain a greater reincorporation of the surviving patients to work, as well as to improve the patients’ quality of life, and to seek their reintegration into a standardized life. Support should be given to the environment, including support in the work environment to facilitate return to work, including measures for partial and gradual return to work and/or job adaptation. It is also necessary to be aware that the woman who survives breast cancer often needs psychological reconstruction and this also means the therapeutic «rest» of the clinical world that attended her in the phase of active disease and reinforced with a period of adaptation and coping of the vital realities.es_ES
